I think Harrow being in a real asylum was meant to be a bit of callback / karmic justice.
If I recall correctly, Khonshu specifically asked Mark to be his Moon Knight at least once.

I got the impression that the asylum is still Mark/Steven's "resolving mechanism" or whatever they had called it. It's the place where Mark and Steven (and later Jake?) can meet face to face and work things out.
I think Harrow was still in there as a figment of their shared imagination, but...
You know, with them now starting to test the waters with including alternate heads, I wouldn't be terribly surprised to see alternate hands becoming an occasional accessory.
The thing is RED COG realizes that. That's his gimmick. He knows that he's the "Ensign Nobody" that beams down with Kirk and Spock. He's constantly terrified that he'll be killed just so that everyone knows that the situation is dangerous.

I was today years old when I found out that there was a second wave of Megaforce toys that never got released. I don't know how many of you remember Megaforce, but it was awesome.
